Abstract

Clinical Pharmacy and Pharmaceutical Care play a significant role in health promotion and the prevention of medication-related errors. Such errors can occur during prescribing, dispensing, administration, or patient use, potentially causing harm to health. In this context, the pharmacist works closely with the patient, providing guidance on the correct use of medications, including schedules, dosages, potential interactions, and adverse effects. Pharmaceutical practice also contributes to identifying treatment-related issues, particularly in patients taking multiple medications simultaneously. Patient education and collaboration among pharmacists, physicians, nurses, and other healthcare professionals are essential for enhancing treatment safety. Thus, Clinical Pharmacy and Pharmaceutical Care should be viewed as vital public health strategies, as they help reduce risks, prevent harm, improve treatment adherence, and promote the safe and rational use of medications. In conclusion, the pharmacist plays an essential role in healthcare, contributing not only to the appropriate use of medications but also to patients' quality of life and safety.
