DRUG-INDUCED PANCREATITIS: A SYSTEMATIC REVIEW OF EPIDEMIOLOGY, DIAGNOSIS AND MANAGEMENT

Abstract

Drug-induced pancreatitis (DIP) is a rare yet potentially serious adverse reaction, accounting for approximately 0.1% to 5% of acute pancreatitis cases. The increased use of medications—particularly psychotropic drugs and incretin-mimetic agents used to treat obesity and type 2 diabetes mellitus—has heightened interest in their pancreatic toxicity. This systematic review aimed to analyze the epidemiological, diagnostic, and therapeutic aspects of drug-induced pancreatitis based on evidence published between 2011 and 2026. Findings indicate potential pancreatotoxicity across several drug classes, notably angiotensin-converting enzyme (ACE) inhibitors, immunosuppressants, antimicrobials, anticonvulsants, benzodiazepines, and glucagon-like peptide-1 receptor agonists (GLP-1RAs). Pharmacovigilance data and observational studies point to an increase in reports associated with semaglutide and other GLP-1RAs, although establishing causality still requires more robust evidence. Diagnosis relies on the presence of compatible abdominal pain, amylase and/or lipase levels exceeding three times the upper limit of normal, and pancreatic abnormalities on imaging, while ruling out common causes such as cholelithiasis, hypertriglyceridemia, and alcohol use. The temporal relationship between drug exposure, symptom onset, and the response to drug discontinuation is crucial for establishing causality. Treatment involves withdrawing the suspected drug and managing acute pancreatitis through hydration, analgesia, nutritional support, and monitoring for complications. In conclusion, early recognition and pharmacovigilance are essential to reduce morbidity, prevent recurrences, and promote the rational use of medications.
